SW03 U18 Curfew Exemption Application
This form can only be used for this purpose:
- live with parents or legal guardian during study period/holiday → Complete Student Details and Section (1).
Application must be submitted three (3) business days prior to the curfew exemption. Late submissions will not be processed and you are required to sign curfew.
A student must continue signing curfew as per U18 Curfew Safety policy when a curfew exemption is not approved. Please have the parent/legal guardian OR caregiver print name on parents' behalf.
